If your app tells someone to lift 300lbs and they snap their spine, can you be sued? Yes.
Your Terms of Service must explicitly state that the app provides information, not medical advice or professional coaching. Force a checkbox acceptance on sign-up. Never use the word "prescribe" in your UI; use "suggest" or "recommend".
